The management of febrile neutropenia (FN) in oncohematological patients is undergoing a paradigm shift driven by a deeper understanding of patients' pathophysiological heterogeneity, the increasing speed and accuracy of microbiological diagnostics, the emergence of new antibiotics, and the incorporation of predictive artificial intelligence (AI) as a tool to support clinical decision-making.

Our objective is to provide an updated and comprehensive overview of the factors influencing antibiotic treatment in FN.

We propose that its management should be based on three essential pillars. First, accurate risk stratification for bacterial infection. Second, ensuring that patients receive appropriate empirical antibiotic therapy, tailoring the initial choice according to the results of surveillance cultures from oropharyngeal and/or fecal microbiota.

Finally, prioritizing antibiotic choices that preserve intestinal microbiota eubiosis as much as possible. Reducing colonization and overgrowth of facultative aerobic/anaerobic flora (Enterobacteriaceae, non-fermenting Gram-negative bacilli, and Enterococcus spp.) while preserving strict anaerobic flora decreases the risk of bacterial translocation and complications such as graft-versus-host disease (GVHD) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) or loss of CAR-T functionality. This decision model, grounded in objective criteria, aims to balance the need for effective empirical coverage with responsible antibiotic use. El abordaje de la neutropenia febril (NF) en el paciente oncohematol gico est experimentando un cambio de paradigma impulsado por una comprensi n m s profunda de la heterogeneidad fisiopatol gica de los pacientes, el creciente potencial de las pruebas microbiol gicas en t rminos de rapidez y precisi n, la aparici n de nuevos antibi ticos y la incorporaci n de la inteligencia artificial (IA) predictiva como apoyo a la toma de decisiones cl nicas. Nuestro objetivo es ofrecer una visi n actualizada y completa de los factores que condicionan el tratamiento antibi tico en la NF.

Proponemos que su manejo debe basarse en tres pilares esenciales. En primer lugar, una correcta estratificaci n del riesgo de infecci n bacteriana. En segundo lugar, garantizar que los pacientes reciban un tratamiento antibi tico emp rico adecuado, personalizando la elecci n seg n los resultados de los cultivos de los frotis de la microbiota orofar ngea y/o fecal. Por ltimo, priorizar la elecci n de antibi ticos que preserven, en la mayor medida posible, la eubiosis de la microbiota intestinal ya que disminuir la colonizaci n y el sobrecrecimiento de flora aerobia/anaerobia facultativa (Enterobacteriaceae, BGN no fermentadores y Enterococcus spp.)

preservando la flora anaerobia estricta, reduce el riesgo de translocaci n bacteriana y de complicaciones como el desarrollo de enfermedad del injerto contra el receptor (EICR) en el trasplante de progenitores hematopoy ticos alog nico (alo-TPH) o la p rdida de funcionalidad del CAR-T. Este modelo de decisi n, basado en criterios objetivos, busca equilibrar la necesidad de una cobertura emp rica eficaz con un uso responsable de los antibi ticos.
